The short answer, for a week with four rounds and mid-range hotels, is about £520 a golfer excluding flights. For a fortnight with eight rounds, about £949.
The longer answer is that green fees in Thailand range from ฿500 (£11) to ฿8,500 (£194) depending entirely on which course you walk onto, and that is the number that moves your budget more than anything else. Here is the whole breakdown, from our own fee data across 62 courses.

Caddies. Compulsory at almost every course in Thailand. Around ฿400 a round plus a tip of ฿300–500. Over eight rounds that is roughly £150 nobody budgets for. A handful of clubs make it optional — Sawang Resort Golf Club is one.
Carts. ฿600–1,000 per cart, usually shared between two. Not always compulsory, but walking 18 holes in Thai heat is a decision you make once.
Transfers. ฿300–800 each way to a course by taxi, or ฿1,200–1,800 for a minibus for a group. Over eight golf days this adds up faster than expected.
Club rental. ฿800–2,500 a set. If you are playing more than three rounds it is usually cheaper to pay the airline to carry your own.

Flights are the other half. From the UK, expect £550–900 return depending on season; from Australia £450–750; from Scandinavia £500–850. Book the right month and you save on both flights and green fees at once.
Three things quietly double a Thailand golf budget.
Playing the famous courses every round. Siam CC - Old Course is a genuinely great course and costs ฿8,500 (£194). Play it once, not four times.
Going in February. Peak season means peak green fees and peak flights. The same trip in May costs a third less.

At almost every course, yes. Budget ฿400 a round plus a tip of ฿300 to 500. A small number of clubs make caddies optional for stay-and-play or long-stay guests, but assume you are paying unless the course says otherwise.
May to October is the green season — lowest green fees, lowest flight prices and afternoon rain rather than all-day rain. Courses are quiet and many publish reduced seasonal rates. The trade-off is heat and humidity.
